Ingestion of raw or undercooked shiitake mushrooms is associated with a distinctive flagellate erythema. We describe a 61-year-old Caucasian man who presented with a pruritic, erythematous eruption of multiple linear streaks on the trunk and extremities starting 1 day after eating raw shiitake mushrooms.

• Bleomycin, an antitumor medication, is used as treatment with certain malignancies. Flagellate erythema has been reported as an adverse effect of bleomycin with an incidence rate of 10-20%.3 The precise mechanism remains unknown although Flagellate erythema is characterized by linear erythematous streaks with hyperpigmentation. The condition is a characteristic side effect of bleomycin, an antineoplastic sulphur-containing antibiotic developed in the 1960s, and might also occur after consumption of raw or undercooked shiitake mushrooms. Shiitake flagellate dermatitis is a toxic reaction to lentinan, found in fresh, powdered, or lightly cooked shiitake mushrooms. Lentinan is a thermolabile polysaccharide that activates interleukin 1 secretion, leading to vasodilation , haemorrhage and rash. First reported by Nakamura in 1977 [ 1 ], ingestion of raw or undercooked shiitake mushrooms can induce flagellate erythema, also known as shiitake dermatitis. The eruption often occurs within the first 2 days after ingestion of the mushroom and is frequently pruritic.

Here we report a characteristic but rare side effect of flagellate erythema, which developed secondary to bleomycin in a 27-year old woman with Hodgkin’s lymphoma after two cycles of treatment with adriamycin, bleomycin, vinblastine, dacarbazine regimen. Shiitake dermatitis is a distinctive rash that can occur following the ingestion of raw or undercooked shiitake mushrooms (Lentinula edodes). It is characterised by pruritic, erythematous, linear streaks that resemble whiplash marks, hence the name flagellate. Shiitake dermatitis is also known as flagellate erythema and toxicodermia. The characteristic flagellate linear groups of erythematous vesicular or urticarial lesions may occur from a few hours to 9 weeks after administration of the drug.
